Simple Marketing PHP Contact Form v2 with Skins

The Simple Marketer contact form is a simple, easy to use contact form that you can include on your webpage. It allows the user to enter their name, email address, phone number, subject, message and select how they want to be contacted back. It also allows the user to select marketing information such as what industry they're involved in, what products/services they're interested in and where they found out about your website. It then sends this information to your email address via PHP. Key features of this contact form are:

- Easy to configure, using an included XML file. - Change the color of the background, or use one of the 38 skins included. - Change the text and color of the title, field names and error messages. - Change the required fields. - Change the items listed in the three marketing combo boxes using the XML file. - Easy to install: insert the SWF file into your webpage, upload along with the PHP and XML files and that's it! No need for you to have Flash installed (unless you want to edit the source file). - Validates the fields before sending, to make sure that information has been entered into each field, and that a valid email address has also been entered. - Includes the sender's IP address, to help identify senders of unwanted messages.
